Whilst each LEA has their own Provider Statement, we find most LEAs use the same format. Therefore, below you can find a generic Provider Statement for Adaptable Travel which in most instances, is sufficient for LEAs. If you require any further information or different forms completing, please contact us and we will be more than happy to assist.
